New Patients
Need to Establish with us as your new Primary Care Provider?
At Merit Medical Care, we understand how stressful establishing healthcare can be. Here's a quick and easy list of what is needed for your first appointment:
1. After you've verified we take your insurance, call the customer service number on your insurance card and ask to change you PCP to "Dr. San Thiha".
2. Print or write a list of your medications and the dosages for each.
3. Fill out the New Patient Paperwork:
There's a couple options for this step...
a. Stop by during office hours to pick up the paperwork to fill out at home and bring to your appointment
b. Print out the paperwork that is under our "Appointments" tab or click below to fill out and bring to the appointment
c. Come to the appointment and fill it out before being seen
4. Have your Driver's License and Insurance Card ready to show at the appointment
5. Call and schedule your appointment down below!
Existing Patients
Coming for your next visit?
If you are an existing patient, here are a couple items to note before your next visit:
1. All patients 18-64 years of age must fill out an annual mental health screening at their appointment (PHQ-9)
2. All patients 65 years of age or older must fill out an annual mental health screening at their appointment (HRA)
3. If there are any changes in address, phone number or insurance, please let the front desk know at the time of your appointment
